If you would like to make an appointment, please call our office @ (408) 871-WELL or email us . We strive to accomodate all new patients with the first available appointment and flexible new patient times.
Financial:
We will be more than happy to submit insurance forms on your behalf and help you recover the most from your benefits. We will do everything we can to help you afford the care you need. We have cash plans to make care more affordable.
We gladly accept cash, check, Visa or Mastercard.
Cancellation Policy:
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment slot to another practice member. A fee may be charged if a patient does not show up for an appointment without this courtesy notice.
